Case Manager Details:

• Flexible Hybrid Schedule – Work a mix of in-home visits and remote after training
• Competitive Salary – Starting at $42,000 – $50,000 based on experience
• Work-Life Balance – Monday-Friday schedule with only one Saturday per month

Case Manager Job Description:

• Provide critical in-home support to at-risk families and individuals
• Conduct home visits, complete family assessments, and develop service plans
• Advocate for clients by connecting them with essential community resources
• Collaborate with school personnel, DHS social workers, and service providers
• Offer flexible support, including occasional on-call availability, to meet client needs
• Participate in staff meetings, training sessions, and case reviews

Case Manager Job Requirements:

• At least two years of experience in case management or a related field
• Experience navigating public benefits for clients
• Previous experience working with school personnel is a plus
• Recent college graduates encouraged to apply!!
• Bachelor’s degree required
• Mandated Reporter Certificate (valid within five years)
