The Regional Director of Care Management will be responsible for the strategy, function and operations of all case management and utilization management activities serving Loyola Medicine Illinois hospitals in alignment with Trinity Health Case Management and Utilization Management. This position will report the CNO and RVP of Care Management and is accountable to Trinity Health system office relative to adherence to Trinity policies and procedures. They serve as an active member of the Regional and local health ministry leadership teams with the principal goal of providing Case Management and Utilization Management services for patients, visitors, colleagues across the Region.
Position responsibilities:
- Develops and monitors processes to ensure alignment with Trinity Health values and regional culture.
- Plans, directs, controls, and evaluates the activities, functions, and management of personnel within the Region. Serves as the key contact with Trinity Health to align Care Management vendor processes.
- Ensures ongoing process improvement within the Care Management scope and partners with Population Health, Clinically Integrated Network and Accountable Care on all longitudinal plan of care initiatives.
- Ensures compliance with all local, state and federal regulatory requirements within the Care Management divisions.
Requirements:
- Master’s degree in nursing or Health related field.
- 6-10 years of previous job-related experience; 10+ years preferred.
- Experience as Utilization or Case Management RN
- 3-5 years as a Director of Care Management
- Current Registered Nurse License State of Illinois
- Certification in Case Management
